CUSTOMIZED FACIAL CONTOURING

ONUL l PLASTIC SURGERY

Creating a Harmonious Face
Considering Overall Facial Proportions

Orthognathic surgery, which corrects the position of the
upper and lower jaws, is a high-precision procedure where
even a 1mm discrepancy can lead to significant differences in results.

It’s about reshaping the jaw and improving skeletal malocclusions like protruding mouths, receding chins, and facial asymmetry. Precise analysis and diagnosis by a specialist are crucial to plan the surgery carefully. At ONUL PLASTIC SURGERY, we perform sophisticated surgeries that address aesthetic and functional improvements, offering safer and more exceptional results than traditional orthognathic surgeries.

Types of Orthognathic Surgery Cases

At ONUL PLASTIC SURGERY, we establish accurate surgical plans through thorough examinations using state-of-the-art equipment and draw upon extensive clinical experience and high-level expertise to avoid unnecessary surgeries and prevent overcorrection

CASE. Prognathism
Commonly referred to as Class III malocclusion, this condition involves excessive lower jaw growth, causing it to be larger or protrude more than the upper jaw. Severe cases may lead to chewing disorders, digestive issues, or temporomandibular joint abnormalities. If the cause is skeletal rather than dental, orthognathic surgery is essential.

Pre-Surgery Description
Orthognathic, square jaw and one-cut chin tip surgery were performed.
CASE. Long Face
The facial width-to-height ratio is about 1:1.3, but a significantly higher ratio indicates a long face. Often accompanied by a protruding mouth or prognathism, orthognathic surgery can be the ultimate solution, especially when the middle face is long, causing excessive gum exposure and an aged appearance.

CASE. Facial Asymmetry
Facial asymmetry is when the left and right sides of the face are unbalanced, usually due to underdevelopment on one side or overgrowth on the other. Severe asymmetry can be corrected with orthognathic surgery.

Orthognathic surgery isn't
always the only answer.
We conduct personalized surgery for each patient based on accurate diagnosis.

If teeth occlusion is standard, surgical methods other than orthognathic surgery can reduce the procedure's cost, recovery period, and burden. Instead of universally recommending orthognathic surgery, we consider the harmony of the facial bones and other areas, providing the most suitable method through thorough diagnosis. Since pre-surgery suitability diagnosis is crucial, receiving an accurate diagnosis from a skilled and experienced specialist is essential.
01

If the teeth’s occlusion is normal, but the jaw appears long and protruding, contour surgery, rather than orthognathic surgery, can be effective for improvement.

Before
02

For a long lower face, surgery to shorten the jaw length can be performed using nerve-preserving techniques that avoid damage to the mandibular nerve, effectively reducing a long face.

Before
03

If the midface appears flat, making the face look broad and long, adding volume with noble surgery, fat grafting, or fillers can improve the appearance of a protruding jaw.

Before
04

For facial asymmetry, contour surgery on one side or a corner lip lift can correct asymmetry. Also, if one side has more soft tissue like fat or muscle, asymmetry can be corrected with tissue removal surgery based on an accurate diagnosis using facial ultrasound.

Before
05

For those apprehensive about protruding mouth surgery and with mild symptoms, fat grafting, noble surgery, or inserting silicone implants at the tip of the front jaw can reduce the prominence of the protruding area around the mouth. Custom 3D implants can create a natural, inherent line.

Mandibular Osteotomy
(Single Jaw Osteotomy)

If the asymmetry or position of the maxilla isn’t significantly adverse, surgery can be limited to just the mandible in orthognathic procedures. This can be a much less burdensome and highly effective option, with a significantly reduced scope of surgery.

At ONUL PLASTIC SURGERY, we don’t adhere to just one method! perform surgery using the method that best suits the patient. After a precise diagnosis of the patient's condition, we choose between SSOR (Sagittal Split Osteotomy) and MRO (Mandibular Ramus Osteotomy) for the surgery.

Rather than a method convenient for the surgeon, we determine the surgery based on the direction of jawbone movement and the position of the nerve line, considering the patient's unique anatomical structure and lifestyle pattern.

O N U L
IVRO (Intraoral Vertical Ramus Osteotomy)
SSRO (Sagittal Split Ramus Osteotomy)

This method involves vertically osteotomizing the mandible's posterior part where no nerve line exists, moving it to an appropriate position, and allowing it to fixate naturally.

Surgical Method

This method involves osteotomizing the center of the lower jaw and pushing the jawbone back, then fixating it with fixing pins.

1. It almost doesn’t affect nerve pathways, minimizing nerve damage.
2. The surgery duration is relatively short, leading to faster recovery.
3. Post-osteotomy, intermaxillary fixation is essential, requiring regular checks.
4. It's effective in alleviating symptoms of the temporomandibular joint and maintaining its function.
Characteristics
1. The wide bone contact surface allows for a quick recovery.
2. Intermaxillary fixation isn’t needed.
3. It provides stability in moving the jaw initially, facilitating a quicker return to daily activities.
4. It's suitable for severe cases of receding chin and asymmetry.

Protruding Mouth Surgery
(ASO - Anterior Segmental Osteotomy)

A protruding mouth is when the teeth and gums in the front portion protrude notably more than the nose or chin tip, even with normal occlusion. Protruding mouth surgery can be simpler than orthognathic surgery yet yield equal or more significant effects. However, overcorrection can lead to an unnatural mouth shape, making the surgeon's experience and expertise crucial.

CASE. Protruding Mouth Surgery

Protruding mouth surgery can be simpler than orthognathic surgery yet yield equal or more significant effects.

Two-Hour (2hour) Drain

At ONUL PLASTIC SURGERY, we perform precise and swift surgeries based on an advanced system and the rich experience of our medical staff, minimizing blood loss and often not requiring drainage in most contour surgeries.

However, in surgeries like bimaxillary osteotomy and extensive mandible reduction, some bleeding is inevitable due to blood pressure increases upon waking from anesthesia. Even a tiny amount of blood, if accumulated, can cause hematoma or infection, so we insert a drain during awakening from anesthesia. This drain exits through the intraoral incision site, eliminating concerns about scarring, and is removed approximately two hours post-surgery to minimize patient discomfort.

Pursuing Naturalness and
Facial Proportion Harmony

The essence of cosmetic surgery is naturalness.
Especially for contour surgeries, unlike general eye and nose surgeries, harmony with the overall body and facial proportions is fundamental.
At ONUL PLASTIC SURGERY, we pursue a natural facial shape that looks like it has always been yours, based on the golden ratio of the face.

Using 3D CT and 3D printing, we create actual-sized models of the current bone structure to produce 1:1 custom implants. This customization allows precise calculation of the patient's bone shape, shortens surgery time, and reduces the risk of infection from modifying the shape during surgery.
We precisely calculate to predict the post-surgery appearance, ensuring the best results.

Post-Surgery Swelling Management Program

At ONUL PLASTIC SURGERY, we assist you in returning to your daily life naturally through methods such as swelling reduction injections and management, based on the physician's assessment, to facilitate rapid elimination of swelling.

  • Swelling Reduction Injections These injections help reduce inflammation and swelling, promote regeneration, stabilize cellular membranes in surgical areas, and prevent aging and degenerative changes.
  • Swelling Management We support blood circulation, aiding in the efficient drainage of stagnant fluids and blood, proving effective against swelling and bruising.
  • Regeneration Management Post-surgery, our approach includes removing dead skin cells and sebum, soothing tired skin, and providing hydration and nutrients for skin regeneration.

Post-Surgery Scar Management Program

Scarring is a significant concern with incisional surgeries.
We spare no effort in minimizing scars throughout the surgical process, and we continue with ongoing scar injections and laser scar management post-surgery.

  • Scar Management Injections These injections aim to minimize hypertrophic scars and keloids while aiding skin regeneration
  • Pigmentation Laser This treatment prevents pigmentation at the surgical site and destroys melanin, effectively lightening the skin. It also reduces the redness of scars and suppresses excessive vascular proliferation.
  • Scar Laser Our laser treatment increases the activity of fibroblasts, promoting dermal collagen production to correct skin textures disrupted by scarring.
