CASE. Prognathism
Commonly referred to as Class III malocclusion, this condition involves excessive lower jaw growth, causing it to be larger or protrude more than the upper jaw.  Severe cases may lead to chewing disorders, digestive issues, or temporomandibular joint abnormalities. If the cause is skeletal rather than dental, orthognathic surgery is essential.

CASE. Long Face
The facial width-to-height ratio is about 1:1.3, but a significantly higher ratio indicates a long face.  Often accompanied by a protruding mouth or prognathism, orthognathic surgery can be the ultimate solution, especially when the middle face is long, causing excessive gum exposure and an aged appearance.

Instead of universally recommending orthognathic surgery, we consider the harmony of the facial bones and other areas, providing the most suitable method through thorough diagnosis. Since pre-surgery suitability diagnosis is crucial, receiving an accurate diagnosis from a skilled and experienced specialist is essential.

If the teeth’s occlusion is normal, but the jaw appears long and protruding, contour surgery, rather than orthognathic surgery, can be effective for improvement.

For a long lower face, surgery to shorten the jaw length can be performed using nerve-preserving techniques that avoid damage to the mandibular nerve, effectively reducing a long face.

If the midface appears flat, making the face look broad and long, adding volume with noble surgery, fat grafting, or fillers can improve the appearance of a protruding jaw.

For facial asymmetry, contour surgery on one side or a corner lip lift can correct asymmetry. Also, if one side has more soft tissue like fat or muscle, asymmetry can be corrected with tissue removal surgery based on an accurate diagnosis using facial ultrasound.

For those apprehensive about protruding mouth surgery and with mild symptoms, fat grafting, noble surgery, or inserting silicone implants at the tip of the front jaw can reduce the prominence of the protruding area around the mouth. Custom 3D implants can create a natural, inherent line.

If the asymmetry or position of the maxilla isn’t significantly adverse, surgery can be limited to just the mandible in orthognathic procedures. This can be a much less burdensome and highly effective option, with a significantly reduced scope of surgery.

At ONUL PLASTIC SURGERY, we don’t adhere to just one method! perform surgery using the method that best suits the patient.  After a precise diagnosis of the patient's condition, we choose between SSOR (Sagittal Split Osteotomy) and MRO (Mandibular Ramus Osteotomy) for the surgery.

Rather than a method convenient for the surgeon, we determine the surgery based on the direction of jawbone movement and the position of the nerve line, considering the patient's unique anatomical structure and lifestyle pattern.

IVRO (Intraoral Vertical Ramus Osteotomy)
SSRO  (Sagittal Split Ramus Osteotomy)

This method involves vertically osteotomizing the mandible's posterior part where no nerve line exists, moving it to an appropriate position, and allowing it to fixate naturally.

Surgical Method

This method involves osteotomizing the center of the lower jaw and pushing the jawbone back, then fixating it with fixing pins.

1. It almost doesn’t affect nerve pathways, minimizing nerve damage.
2. The surgery duration is relatively short, leading to faster recovery.
3. Post-osteotomy, intermaxillary fixation is essential, requiring regular checks.
4. It's effective in alleviating symptoms of the temporomandibular joint and maintaining its function.

2. Intermaxillary fixation isn’t needed.
3. It provides stability in moving the jaw initially, facilitating a quicker return to daily activities.
4. It's suitable for severe cases of receding chin and asymmetry.

Protruding Mouth Surgery
(ASO - Anterior Segmental Osteotomy)

A protruding mouth is when the teeth and gums in the front portion protrude notably more than the nose or chin tip, even with normal occlusion. Protruding mouth surgery can be simpler than orthognathic surgery yet yield equal or more significant effects. However, overcorrection can lead to an unnatural mouth shape, making the surgeon's experience and expertise crucial.
