Recently, “square shoulders” have become a popular beauty trend among people in their 20s and 30s, leading to a growing interest in shoulder filler injections. The goal is to create a straighter shoulder line, improve body proportions, and enhance the way clothing fits. Although often referred to as “square shoulder fillers,” many people are still unfamiliar with the procedure. Is it safe, and what should patients consider before undergoing treatment?

Filling Depressed Areas to Improve Shoulder Contours

Shoulder fillers are designed to add volume to hollow or sloping areas of the shoulders, creating a straighter, more defined shoulder line. People with narrow or rounded shoulders may appear to have a shorter neck or a heavier upper body. Enhancing the shoulder contour can help improve overall body balance and proportions.

According to Dr. Lee Seok Lim of Lim's Theme Dermatology Clinic, the procedure is particularly effective for people with rounded shoulders, narrow shoulders, or so-called "hanger shoulders." In some cases, a small amount of filler may also be injected around the clavicle to create a more sculpted upper-body contour.

The procedure is performed under local anesthesia by injecting filler into the subcutaneous fat layer of the shoulders and typically takes 10 to 30 minutes.

Dr. Insoo Kirk of Onul Plastic Surgery explained that the procedure is generally associated with minimal discomfort, allowing most patients to resume light daily activities immediately afterward. Mild swelling or bruising may occur but usually resolves within one to two days, while the results typically last six months to one year.

Because the treatment is relatively quick and requires minimal downtime, it has become an attractive option for patients seeking non-surgical body contouring.


Side Sleeping Can Affect the Results—Exercise and Posture Correction Should Be Considered First

Although shoulder filler injections are relatively simple, they are not without risks. In addition to temporary pain and bruising, rare complications such as allergic reactions, infection, or the formation of nodules may occur.

Dr. Lim noted that shoulder filler procedures often require 20–50 cc or more per shoulder, making the treated area significantly larger than the face and increasing the risk of infection. For this reason, strict sterile technique and treatment by an experienced physician are essential.

The type of filler used is also important. Dr. Lim recommends hyaluronic acid (HA) fillers rather than permanent fillers or long-lasting collagen fillers. If complications occur, HA fillers can be dissolved with hyaluronidase, whereas permanent fillers cannot be easily removed. Even with HA fillers, however, allergy testing should be considered because some patients may experience hypersensitivity reactions.

Proper Aftercare Is Essential

Post-treatment care plays an important role in achieving good results. Since the shoulders are constantly moving and frequently exposed to external pressure, the injected filler may shift if not properly protected.

Dr. Kirk advises patients to avoid sleeping on their side, as prolonged pressure may compress the filler and alter its shape. Patients should also avoid activities that heavily involve the shoulders—such as swimming or weight training—for approximately two weeks after treatment.

Because hyaluronic acid fillers gradually absorb water and settle into the surrounding tissues after injection, the early healing period is particularly important. During this time, physicians may prescribe antibiotics to reduce the risk of infection, and patients are generally advised to avoid alcohol, smoking, and saunas.

Not Everyone Is an Ideal Candidate

Despite its popularity, shoulder filler treatment is not suitable for everyone.

Dr. Lim explained that patients with bleeding disorders should generally avoid the procedure. During injection, cannulas or needles may injure blood vessels, increasing the risk of prolonged bleeding or severe bruising. Because shoulder filler treatments cover a relatively large area and require multiple injection points, the bleeding risk may be even greater than with facial filler procedures.

People with conditions such as hemophilia, thrombocytopenia, or those taking anticoagulant medications should consult their physician carefully before considering treatment.

Dr. Lim also emphasized that shoulder fillers should not be viewed as the first option for minor cosmetic concerns.

"If the goal is simply to make the shoulders look slightly better, I generally do not recommend shoulder fillers. Unless the shoulder shape causes a significant cosmetic concern that affects daily life, improving posture and strengthening the shoulder muscles through exercise is usually the better approach."
