A person's overall impression is influenced not only by the eyes and nose, but also by the proportions of the midface, the shape of the mouth, and facial expressions. Two of the most common lower-face cosmetic procedures for improving facial appearance are philtrum reduction surgery and corner lip lift surgery.

Recently, Korean entertainer Maeng Seung-ji (40) shared before-and-after photos after undergoing both procedures. She explained, "About five years ago, I started feeling that my philtrum looked too long. After the surgery, my philtrum became shorter, and I think I look younger."

Philtrum Reduction Surgery

Philtrum reduction shortens the distance between the nose and upper lip to improve the proportions of the midface.

The procedure can be performed by removing a small amount of skin or muscle beneath the nose, or through an incision inside the mouth, where the upper lip muscles are lifted upward to minimize visible external scarring.

A shorter philtrum helps balance the proportions of the midface and lower face. It can also allow more of the upper teeth to show naturally when smiling or speaking, creating a younger and more refreshed appearance.

However, not everyone is an ideal candidate. According to Dr. Kwak In-soo of Oneul Plastic Surgery, "Facial length is determined by both soft tissue and the underlying facial skeleton. Since philtrum reduction only shortens the soft tissue, it may not be suitable for patients whose facial length is primarily due to a long skeletal structure."

Corner Lip Lift Surgery

A corner lip lift elevates drooping corners of the mouth to create a brighter, friendlier facial expression.

The procedure involves making small incisions at the corners of the lips, weakening the muscles that pull the mouth downward while repositioning the muscles that lift the corners upward.

As a result, the corners of the mouth remain slightly elevated even when the face is at rest, giving a more positive and approachable appearance. The procedure may also help improve lip asymmetry and reduce dark shadows around the corners of the mouth.

Careful Evaluation Before and After Surgery Is Essential

Because both procedures involve highly mobile and visible areas of the face, proper postoperative scar care is especially important.

Dr. Kwak emphasized, "To reduce the risk of inflammation after surgery, patients should carefully follow postoperative instructions, including taking prescribed antibiotics, avoiding smoking and alcohol, and eating soft foods. When philtrum reduction is performed through an intraoral incision, external scarring is minimized, but careful wound care inside the mouth is still necessary."

Most importantly, thorough consultation before surgery is essential to minimize complications and achieve satisfying results. Understanding each patient's unique facial anatomy and aesthetic goals, followed by a personalized surgical plan developed with an experienced plastic surgeon, is key to obtaining the best outcome.
